Collage Rehabilitation Partners Job Description Position Title: Life Skills Trainer Reports To: Operations Manager (or equivalent) FLSA Status: Non-Exempt Job Summary This position is responsible for the application of client rehabilitation strategies, and maintains responsibility for providing minimum to maximum assistance and/or supervision to promote our clients’ success in tasks associated with daily living, recreational activities, and vocational pursuits. Essential Duties & Functions:

  • Assists, guides, and encourages independence when working with clients in the areas of activities of daily living, instrumental community-based living skills, and ongoing rehabilitation and supported living goals and outcomes. These areas of activities can include, but are not limited to, medical and health related issues, medication management, physical activities, cognitive activities, productive activities, therapeutic activities, recreational/leisure activities, and vocational or avocational activities.
  • Supports clients in difficult situations (e.g., refusal of activities, medications, and strategies).
  • Identifies changes in clients’ behavioral, medical, emotional, cognitive, and physical condition and alert applicable team members for appropriate care as needed.
  • Advocates for client needs and expectations through consistent communication with the treatment team.
  • Interacts and communicates with clients, families, and visitors within the program and in the community in a respectful, therapeutic, and professional manner.
  • Interacts and communicates with leadership and coworkers in a respectful and professional manner.
  • Adheres to scheduled activities and completes ongoing/routine assignments with clients as assigned.
  • Displays flexibility and willingness to respond to changes in operations or program needs by changing program assignments when necessary or assuming other duties as assigned.
  • Develops relationships with residents that are respectful, based on mutual trust, and that maintains professional boundaries.
  • Addresses behaviors proactively and respectfully, following company policy and procedures, and follows any behavior management plans in effect for specific residents.
  • Is knowledgeable of clients’ medication regimes and assist in administering client medications as assigned.
  • Seeks out information regarding client’s program and company information before start of shift (e.g. email, staff comm., rounds summaries, meeting minutes, client strategies, postings, memos).
  • Provides a safe, well-organized, clean home-like living environment for residents as per standards for licensure requirements.
  • Monitors and prepares well-balanced meals daily according to resident’s nutrition plan.
  • Transports residents on errands, appointments and outings designed for groups or individuals.
  • Assists residents with medical appointments as requested.
  • Assumes responsibility and accountability for their actions and decisions.
  • Accepts feedback and incorporates feedback information into appropriate action.
  • Maintains strict confidentiality for all resident information and facility operations.
  • Completes all necessary documentation to assure compliance with all state and federal regulations, CARF standards and Collage standards. These may include, but are not limited to: daily data sheets, shift summary sheets, goal sheets, flow sheets (i.e. vitals, weights, and seizure), supervision checklists, and incident reports.
  • Documents activities related to resident’s individual goals in accordance with their car, rehabilitation and/or behavior plan.
  • Demonstrates responsible work habits as evidenced by, but not limited to: appropriate use of time clock, arriving to work in a timely manner, using identified call out systems. Education & Experience Requirements
  • Bachelor's degree in healthcare related field is preferred; minimum of high school diploma or equivalent experience is required.
  • Possession of a valid driver's license, insurance, and an acceptable driving record must be maintained at all times.
  • Competence in writing, planning, organization and documentation, as well as time management skills are required.
  • The ability to communicate effectively with families, professionals, clients, and co-workers is necessary.
Physical Requirements & Working Conditions:
  • Regularly required to communicate, both orally and in writing; occasionally required to speak on the telephone for extended periods of time.
  • Occasionally required to sit; regularly required to bend, stoop, and kneel; frequently required to stand and walk.
  • Regularly required to use hands to type, grasp, handle, or feel; reach, push or pull with hands and arms; and talk or hear.
  • Specific vision abilities required include close vision, color vision, and ability to adjust focus.
  • Regularly required to demonstrate a high degree of attention to details, quality standards and accuracy.
  • Must have the ability to handle and organize multiple priorities and deadlines.
  • Must have the ability to travel and/or operate a vehicle for up to 20% of the time spent working in this position.
  • Must have the ability to occasionally lift, push, or pull 50 lbs. or more.
  • Must be free from a medical condition, including drug or alcohol addiction, which would limit direct care staff persons from providing necessary personal care services with reasonable skill and safety.
  • Collage has determined that all staff are at risk for exposure to potentially infectious materials based on the duties entailed in working with clients or in an office setting. This position has been classified as a role within the organization that may incur occasional occupational exposure based on the job’s duties and responsibilities.
